Our client a newly merged multi-campus health system that includes award winning programs in the Capital District/Albany area of NY, has an established and respected PACE program (Program of All Inclusive Care for the Elderly) that is seeking a full time Internal Medicine/Geriatric Physician to join its team in early 2012. This is an excellent opportunity to join a unique practice environment which provides physicians the opportunity to work with an interdisciplinary team to support frail seniors who remain living in the community and thus avoid nursing home placement. It is a program driven by relationships- not the number of encounters. The physician follows the participant through all care settings: the medical office, at home, in the hospital, and in a nursing home should placement be needed. Physicians interested in applying for this unique opportunity should be board certified in Internal Medicine and/or Geriatrics and have a commitment to compassionate care that is team and family oriented. Strong communications skills are a must. Experience with caring for the elderly is strongly preferred. The Capital District of NY is a medium sized area offering all the amenities of a larger urban area in a beautiful, scenic, and affordable setting. The region has excellent year-round outdoor recreation, including great golf, water-sports, camping, hiking, and great skiing. It offers a wealth of cultural offerings and activities, including several renowned museums and theaters, fine dining, and a year-round events calendar of music and sporting events.
