We are currently seeking a highly qualified can! didate who aspires to be a leader in advancing primary care and the patient-centered medical home in our off-site Outpatient Specialty Center. Besides providing direct patient care, you will also supervise and participate in the teaching of Medical Residents. As the lead clinician in our Patient Centered Medical Home (PCMH), you will work with a team of Nurse Practitioners, a Mental Health Nurse Practitioner, Medical Residents, Registered Nurses, Case Managers, Licensed Practical Nurses, Patient Care Technicians, Clerical Staff, and a Center Administrator to provide comprehensive services to your patient. As part of our multispecialty medical group practice that receives over 33,000 visits each year, care coordination for your patients are easily managed with an extensively implemented EMR, allowing you and your patient a high success rate in reaching all goals.
You will report to the Chairman of Ambulatory Care. Successful candidates will have relevant work experi! ence as well as a proven track record for success. Above all, ! you are deeply committed to our mission of strengthening the healthcare system built on the principles of the medical home.
Position requires:
Doctorate Degree in Medicine from an accredited college
Board Certified in Internal Medicine.
Possess a current unrestricted New York State license as a physician.
Must hold a current and valid BLS certification and will be ACLS certified within 90 days of employment.
Prior experience in working in medical clinics and supervising and teaching the residents is highly desirable.
