Post acute Care Physician We have a unique opportunity for a Board Eligible/Certified Internist.to work for a dynamic organization that is at the forefront of Geriatric care and education in Westchester county, New York. Sarah! Neuman Center is the Westchester campus of Jewish Home Lifecare. A 300 bed, rehabilitation and long term skilled nursing care facility in Mamaroneck, N.Y., that offers nursing home care, a full range of post-surgical, disease recovery and short-stay rehabilitation, an adult day program and respite care. The clinician will work collaboratively with a Nurse Practitioner and other members of our Interdisciplinary Team.
You will be responsible for admissions, discharges, and management of acute and chronic medical problems on a post acute care unit. Skills/Requirements MD Degree from a medical school recognized by the NYS Board of Regents Current NYS License Registration in good standing Board Certified /eligibility interna medicine required. Hospital and/or skilled nursing facility experience required
