We ’re looking for a Registered Nurse who shares our passion for providing holistic long-term care to seniors, people with disabilities, and people recovering from acute health conditions. Vi! siting clients and their families in their homes in New York City, the Registered Nurse is responsible for admitting, re-certifying, and discharging clients from home health care services. The Registered Nurse also provides supervision and mentorship for our Care Partners, who work directly with clients day-to-day. Responsibilities Perform the initial home care visit to assess clients ’ needs and discuss our services Work with clients, their families, their doctors, and other health professionals to create Plans of Care Monitoring clients ’ health, re-evaluating their needs and updating their Plans of Care when needed Supervising Care Partners (including home health aides and LPN ’s) to ensure clients receive excellent care Participating in the on-call schedule Helping create, refine, and implement our o! rganization ’s policies and procedures for home care service! s Maintaining client records and completing any necessary documentation as needed Qualifications For consideration, candidates must be a Registered Nurse in New York State with a current license. Prior experience in home care is preferred but not required; the Registered Nurse will receive mentorship and supervision from our Medical Director, a board-certified geriatrician. Candidates also should have high empathy, be flexible team players, and enjoy working with diverse populations, including the LGBT community. Integrity, reliability, and the ability to work independently are also essential for this role.
Salary " Benefits This position offers flexible working hours. Initially, the Registered Nurse will work on a part-time basis (fewer than 20 hours per week). Strong performers will have the option of working full-time. We offer a competitive salary commensurate with background and experience.
