Excellent opportunity for a full-time BC/BE Geriatrician or Internist with ! Geriatrics experience and an interest in joining a progressive and expanding PACE program. Independent Living Seniors (ILS) is one of the most mature PACE organizations in the nation with over 21 years of experience serving frail seniors in a comprehensive and captivated delivery model of care that relies on an interdisciplinary Care Team to Assess, Plan and Deliver all care needs of its enrolled population. ILS is part of Rochester General Health System located in Rochester, New York. The ideal candidate will have an interest in working collaboratively in a PACE program with an interdisciplinary team to provide gold standard care planning and delivery, inclusive of care in the PACE Center, hospital, home and nursing home. In addition, the candidate should have an interest in teaching.
The innovative Independent Living for Seniors (ILS) program at Rochester General Health System gives the elderly an alternative to nursing home placement. It is designed to enable pa! tients to live in their own homes while being served by an int! erdisciplinary team of supportive services including, but not limited to: adult day care; primary care; nursing rehabilitative therapy and social work services, laboratory; x-ray and ambulance services; nutrition and support services; medical specialty services; skilled nursing facility care; pharmaceuticals, supplies and equipment, acute hospital care; in-home services and transportation.
